This stopped working at Lyx V2.?? or perhaps it was Miktex2.9 . The problem appears to be that Miktex does not setup a repository for automatic downloads when installed anymore (even though package manager can download manually). LyX allows the configuration of menus and toolbar via .ui files. They are found in your system directory (in the ui folder). Look at the files in the lib/kbd directory in the LyX distribution for examples. As we mentioned briefly last issue, LyX now has a bug tracker at bugzilla.lyx.org.
